About the Resource and Referral Program:

Services are provided to individuals and families who need help meeting basic needs: food, clothing, shelter, and utilities.  Applicants qualified for the program will complete a brief assessment with a case manager in-person. Referrals are made to other agencies when appropriate.  Services are offered by appointment only. 

We are moving from a transactional program (short-term, one-time interactions) to a relational stabilization model (long-term, deeper engagement).

  • Each region requires brief case management to those requesting assistance.

  • Each region has specific funding sources which determine the type and amount of financial assistance we are able to provide.

  • All individuals will be provided with an opportunity to receive a review of their budget and learn ways to improve their financial situation. This may include learning about other resources for their specific situation.

  • Referrals for other Catholic Charities’ ongoing case management programs may be provided based on eligibility and interest.

  • Services are offered by appointment only.

If you are a resident of DeKalb, LaGrange, Noble, and Steuben County only and in need of assistance, please fill out each section of this form. These questions will help determine if your family may qualify for our Resource and Referral program. 

We cannot process your request without all of this important information, so please answer every question included on the application form. 

Please be prepared to provide proof of:

  • Household income

  • Identification

  • Copies of lease agreements/ bills/invoices
